Can a cat scratch give you rabies?

Updated By PetymFacts checked against official sources on

Quick answer

Yes, it can. Rabies spreads through saliva, and India's National Guidelines for Rabies Prophylaxis list scratches alongside bites as ways in. Cats cause about 2% of human rabies in India. Wash any scratch with soap and running water for up to 15 minutes, then see a doctor or a government anti-rabies clinic the same day.

The doctor decides on treatment from the type of scratch: a graze without bleeding is category II, and a scratch that breaks the skin and bleeds is category III. If the cat is your own and can be watched, keep it at home for 10 days and tell the doctor how it is.

What should you do after a cat scratch?

The same steps for a kitten, a pet cat or a community cat. Start within minutes, finish at a clinic today.

  1. Wash it now

Flush the scratch under running water and wash it with soap. The NCDC guidelines say washing is desirable for up to 15 minutes, then an antiseptic such as povidone iodine. National Centre for Disease Control, Ministry of Health and Family Welfare

  2. Put nothing else on it

No turmeric, chilli, oil, lime or herbs. The guidelines say these give no advantage and the irritation may help the virus reach the nerves. National Centre for Disease Control, Ministry of Health and Family Welfare

  3. Note what happened

Did it bleed? Did the cat also bite or lick a cut? Is it your cat, a neighbour's or a street cat? The doctor needs this to place the exposure in a category. National Centre for Disease Control, Ministry of Health and Family Welfare

  4. See a doctor today

Any doctor, or a government hospital's anti-rabies clinic. Anti-rabies vaccine and serum are free in public health facilities under the National Free Drug Initiative. National Centre for Disease Control

  5. Watch the cat for 10 days

If it is your cat or one you can see daily, keep an eye on it for 10 days and report any illness or odd behaviour. This period is valid for dogs and cats only. National Centre for Disease Control, Ministry of Health and Family Welfare

How can a scratch pass on rabies?

Rabies lives in saliva, and cats lick their paws.

The NCDC guidelines say the rabies virus passes to people through an animal's saliva, after bites, scratches, licks on broken skin and contact with the lining of the eyes, nose or mouth. The European Advisory Board on Cat Diseases says the same of cats: the virus is shed in saliva a few days before signs appear and spreads through a bite or a scratch of the skin or mucous membranes.

Dogs cause most cases in India, about 95% according to the NCDC, but cats are next at about 2%, followed by jackals, mongooses and others. The WHO says rabies is virtually 100% fatal once symptoms appear, and that deaths are preventable with prompt treatment. That is why a scratch is not something to wait and see about.

A rabid cat is often not hard to spot, but you cannot rely on that. The ABCD guideline says nearly all rabid cats show the furious form, with aggression, and that the average incubation period in cats is two months but can range from two weeks to many months. A cat can be shedding the virus before it looks ill.

Which kind of cat scratch needs which treatment?

Indian doctors use the WHO's three categories. This table puts common cat contacts into them, as the guidelines describe each category.

CategoryWhat the guidelines recommend
Stroking or feeding a cat, or a lick on unbroken skin World Health OrganizationINo treatment beyond washing, if the history is reliable
A graze or minor scratch with no bleeding, or a nibble on bare skin National Centre for Disease Control, Ministry of Health and Family WelfareIIWound care and rabies vaccine
A scratch or bite that breaks the skin, or a lick on a cut National Centre for Disease Control, Ministry of Health and Family WelfareIIIWound care, rabies vaccine and rabies immunoglobulin
Saliva in the eyes, nose or mouth National Centre for Disease Control, Ministry of Health and Family WelfareIIIWound care, rabies vaccine and rabies immunoglobulin

Only a doctor places your exposure in a category. When in doubt, the guidelines say treatment should be started.

Does it matter if the cat is my own vaccinated pet?

It matters less than you might hope. The NCDC says animals vaccinated against rabies do not get or pass on the disease, but vaccines can fail because of how they were given, the dose, the vaccine's quality or the animal's health. Without a laboratory antibody result, a vaccinated animal cannot be presumed protected, so the guidelines say treatment should be given irrespective of the animal's vaccination status. Take the cat's vaccination card to the doctor anyway: it is part of the history the doctor weighs.

The 10-day watch can change the plan. The NCDC explains that an infected animal usually shows signs, or dies, within 1 to 7 days, which is the basis for observing dogs and cats for 10 days. If the cat stays healthy, the doctor may modify the treatment, depending on the type of course. Treatment still starts on the first day, and only the doctor decides whether to change it. A street cat that cannot be found again cannot be observed, so the doctor will usually plan on that basis.

How do you lower the risk of scratches at home?

Do

Don’t

Is there anything else a cat scratch can cause?

Yes: cat scratch disease, a bacterial infection that is quite separate from rabies. The CDC says it spreads through scratches from domestic or feral cats, particularly kittens, especially when the scratch is contaminated with flea droppings, and through bites or licks on an open wound. It typically causes a small bump at the scratch, a low fever and tender, swollen lymph nodes one to three weeks after contact with a cat. People with weakened immune systems are at higher risk of severe illness.

If a scratch becomes red, swollen or painful, or glands near it swell in the weeks after, see your doctor and mention the cat. And if your own cat scratches more than usual, or a gentle cat starts lashing out, take it to your vet: pain and illness change how cats behave.

Questions owners ask

Do I need an injection for a small cat scratch with no blood?

Probably, so see a doctor. India's guidelines class minor scratches or abrasions without bleeding as category II exposures, for which wound care and rabies vaccine are recommended. Wash the scratch with soap and running water for up to 15 minutes, then go to a doctor or anti-rabies clinic the same day.

Can a kitten scratch give rabies?

It can. Rabies spreads through saliva on a bite or scratch, and the guidelines make no exception for young animals. Kittens also carry the bacteria behind cat scratch disease more often, according to the CDC. Wash the scratch well and see a doctor the same day, whatever the kitten's age.

My indoor cat has never been outside. Do I still need treatment?

Tell the doctor exactly that, with the cat's vaccination card. India's guidelines still treat a scratch that breaks the skin as an exposure, and the doctor decides on treatment. If the cat can be watched for 10 days and stays healthy, the doctor may modify the course.

How soon after a cat scratch should I get treatment?

The same day. The NCDC says treatment should start immediately after exposure, but also that people who come late, even months later, should still be assessed and treated as if the exposure were recent. Do not skip it because time has passed.

Can a cat that looks healthy have rabies?

For a short time, yes. The European Advisory Board on Cat Diseases says the virus is shed in saliva a few days before signs appear. This is why India's guidelines start treatment straight away and use a 10-day watch of the cat to decide whether the course can be changed.
